How soon can I get an appointment?

Our office strives to make every effort to accommodate all appointment requests.  We have convenient hours, with walk-in and urgent care services.  Extended office hours are available by appointment only to accommodate before and after school/work.

What should I bring with me for the appointment?

If you are a new patient/family to our practice, you will need to bring with you any and all:
• Medical records, test results, and lab reports that pertain to the medical condition we are seeing you for
• A list of vaccinations
• A list of all current medications with dosage amounts and pharmacy information for prescriptions
• A list of any providers or specialist that you would like us to contact.
• Your Drivers License and insurance cards 

Copays and Deductibles are required at the time of services.

As required by your insurance carrier.

How long will my appointment take?

Length of appointment times vary. For new patients, we recommend arriving 15 minutes early to fill out paperwork and to process insurance information. Feel free to download our forms by clicking on the green highlight word “paperwork” and complete prior to arriving. This may speed up the check-in process. Wait times will vary depending on the provider you are seeing, the day and the complexity of your illness.

Which provider will I see?

When you call for either a well or sick visit, you will be asked which provider you would like to see. We will do our very best to accommodate your request. Our providers strive to handle all patients’ health concerns with comparable consistency of care. It is our hope that as a patient of Phoenix Healthcare Clinic you will feel comfortable with any one of our providers. We do recognize that you may have a preference for one provider over another and we will always do our best to honor patient requests, as long as there is an available appointment time for the requested provider. If the provider you prefer is unavailable, we will inform you and offer an alternative date or provider.

What type of insurances are accepted?

It is the patient’s responsibility to verify and understand their insurance policies.  Phoenix Healthcare Clinic accepts most major health plans. However, we recommend that you check with your insurance carrier’s Customer Service Department to verify if we or Dr Revels are considered in network providers.  We frequently add new insurance plans, so check to see if we are a participating provider with your plan, or feel free to request us to add your plan if we are not.  If you have an HMO, please contact your carrier to identify your assigned primary care provider.

What if I do not have insurance?

Phoenix Healthcare Clinic wants all patients to have affordable access to healthcare. We will not turn away a patient because they do not have insurance. We offer appointments to those without insurance at a very reasonable rate. “Self-pay” patients are required to pay for the services at the time the services are provided. Our “self-pay” rates are drastically lower than area urgent care or emergency room fees. Please contact us at 913-851-0500 to discuss your situation and to make an appointment.

If I need a procedure, will it be performed in the office?

Minor procedures that do not require general anesthesia may be performed in our office. Types of procedures that may be performed in the office include: sutures, skin biopsy, IUD placement/removal and abscess I&D.

What if I need a form completed by my provider?

Depending on the type of form, completion can be done with 24 hours notice. Due to the complexity of FMLA forms, please allow 3-5 days for completion. In addition, Letters of Medical Necessity may also take an extended amount of time to complete.

How long will it take for a prescription refill?

For medication refill request, please contact your pharmacy. Your individual provider must review and approve all refill requests.  To ensure he or she is available please make your request 48-72 hours in advance.  Refill requests received after 2 pm on Friday will be processed the next clinical business day.  Refills will not be filled after hours, as the on-call provider does not have access to your medical records. Refills will not be filled for those patients who are not current or who require follow up visits or routine labs.

Why do you require photo ID?

Effective 11/1/2011, Phoenix Healthcare Clinic requires valid photo identification issued by a local, state, or federal government agency. This is a requirement mandated by the Federal Trade Commission (FTC) as part of the Red Flags Rule. Please see the FTC website (www.ftc.gov) for more information about the Red Flags Rule and Consumer Protection.

Do you have an Immunization Policy?

We DO NOT have an immunization policy. Our Providers believe strongly in the benefits of routine vaccination. However, we will not exclude children from our practice if they do not follow the recommended vaccine schedule. We believe that all children have the right to adequate healthcare regardless of their vaccination status. Instead we will educate parents and work with them to create a plan that is comfortable for their family. The providers of Phoenix Healthcare Clinic WILL NOT sign a medical exemption required by the state for school entry. Currently, we are NOT able to offer children less than five years of age routine immunizations. Please see our list of offered vaccines under the service tab. We recommend the Johnson County Health Department to our patients who need updates on their vaccinations after well visits.
